Transferring records from a ForHealth practice
This applies if you are:
- Requesting your own medical records
- Requesting a child’s medical records (parent or legal guardian)
- Requesting records for a deceased patient (authorised POA or Executor)
Third-party requests
For authorised third parties requesting records on behalf of patients or providers, including insurers, solicitors, government agencies, or other authorised organisations.
What we need to process your request
- A completed online request form with Signed consent and proof of identity (which will require your signature and consent, as well as proof of identification)
- Payment of an administrative fee (Our team will contact you to confirm the fee)
- Please allow up to 5 weeks for processing
